The HA Injury Report Form is attached to assist players who are injured in Masters games record information they may later require should they need to lodge a sports injury claim through the HA Insurance Scheme. The Association recommends any injured player or official complete the form as soon as possible after the incident so that all information is readily available. The form is NOT submitted to the Association or HA.
